Registering with the surgery
To register with Sutton Coldfield Group Practice:
- Fill out an online registration form
- Visit one of our surgeries after 10am during opening hours to collect a paper registration form.
Find your NHS number
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Named accountable GP
All our patients have an allocated named GP, which is your registered GP. If you wish to know who this is, please ask at reception.
Please note that you can still choose to see the GP of your choice.
